  1. ;;The Indian Health Service (IHS) is working to improve the
  1. ;;quality of its hardcopy and automated medical records. An
  1. ;;important element of the medical record is the Social Security
  1. ;;Number (SSN). SSNs Help IHS to more quickly and accurately find
  1. ;;specific medical records and to unduplicate medical records.
  1. ;;The result is better patient care since an SSN allows IHS and
  1. ;;tribal contract employees to keep a better record of a) a
  1. ;;patient's health history, b) what medicines a patient has taken
  1. ;;and should take, c) what health treatment a patient was given
  1. ;;and should get, d) patient allergies (if any) and e) follow-up
  1. ;;care if needed. A SSN also allows tribal contractors to more
  1. ;;effectively bill third parties ( e.g., insurance companies,
  1. ;;Medicare, and Medicaid) for care provided to patients with such
  1. ;;coverage.
  1. ;;
  1. ;;In order to ensure that it has complete and accurate SSNs on
  1. ;;file, IHS uses information that is available from other
  1. ;;government organizations. The Social Security Administration
  1. ;;and Medicaid are IHS' principal sources of SSN information. The
  1. ;;SSN listed below was provided to IHS. We believe it is your SSN
  1. ;;since the name, date of birth, and sex associated with the SSN
  1. ;;match the information you provided to us.
  1. ;;
  1. ;;Please review the SSN that is listed below and let us know if
  1. ;;it is incorrect. Thank you for helping IHS do a better job for
  1. ;;you.
  1. ;;

  1. ;; Sincerely,
